A family member is asking about a patient's condition

This depends on what the patient has authorized. When in doubt, say nothing.

    Check for patient authorization

    Look for a signed authorization or patient directive in the chart naming this person as someone who can receive information.

    If the patient is present, ask them

    If the patient is with you, simply ask: "Is it okay if I share this information with your family member?" Document their response.

    If no authorization exists, share nothing

    Politely say you are not able to discuss another person's health information. You do not need to confirm or deny the person is even a patient.

    Offer a path forward

    Suggest the patient contact you directly or sign an authorization form if they wish to designate this family member.

Important

Exception for incapacitated patients: If a patient cannot make decisions for themselves, a healthcare proxy or legal guardian may have rights to their information. Ask your privacy officer.
